- Development and review of systems and processes utilised with incident management
- Facilitate incident investigations to identify root causes, contributing factors and failed barriers
- Identification of clearly defined corrective actions that link directly to the root causes and casual factors with agreed timeframes
- Experienced in a range of tools (i.e. ICAM, Fault Tree, The 5 Whys, Fishbone)
- Detailed review of closed out incident investigation recommendations and actions to ensure deficiencies have been adequately addressed to prevent reoccurrence.
